Step 1: Open the Order Form

Step 2: Fill it out

Enter your name, address, etc. at the top of the order form

Switch to the web page and highlight the entire line of the plate you want by dragging with the mouse

Select Edit, Copy or press "Control+C" to copy the line

Switch to the Order Form and click on the first column under "State" line 15 (A15)

Select Edit, Paste Special ...

Press OK to paste as HTML code.

Your plate appears in the order form with the price in the price column

Repeat for each plate. One plate per row. Insert additional rows in the order form with Insert, Rows

Enter the Shipping, Insurance and Sales Tax amounts

Save the file to disk.

Step 3: Send it in

As an e-mail attachment to:davezubal@netzero.net

If your PayPal account is funded by a bank account or has a balance,
Send payment to: davezubal@netzero.net
If your PayPal account is funded by a Debit or Credit Card,
E-mail for the special payment instructions.

Or Print it out and send it snail mail to:

Make payment with Check or Money Order made payable to: D.A.Z. You send cash at your own risk!

We will confirm your order or notify you of any problems (such as plates sold out or postage outside the U.S.) and ship as soon as possible after payment is received.
